Assessment and Recommendations
Our Geriatric Social Worker will evaluate existing abilities and needs through an interview, often with family present. The assessment will include an evaluation of health status, functional status, cognitive functioning, emotional status, environment, legal and financial situation, and family support system.
Service Planning and Coordination
After doing an assessment and making recommendations, the Geriatric Social Worker works with the older adult and/or family to create a plan for addressing needs. Once the plan is in place, the care manager can make specific arrangements to get the services started.
Ongoing monitoring and Coordination
Through visits and phone contacts, the Geriatric Social Worker monitors care and services, provides emotional support, stays informed, and responds as needed. Communication and periodic updates with family members are arranged.
